• Board Certified Emergency Medicine -OR- Board Certified Internal Medicine/Family Medicine with EM experience are welcome to apply

• 24 hours/day of physician coverage (12-hour shifts)

Board Certified in Emergency Medicine -OR- Family Medicine/Internal Medicine with Emergency Medicine experience - REQUIRED
Employment experience in Emergency Medicine of at least 12 months/2,000 hours (not including training) - REQUIRED

OH License (in hand or in process) - REQUIRED

List of procedures the provider is experienced in performing (note in "Experience" field) - REQUIRED

Proof of COVID Vaccination Status (upload to Documents) - REQUIRED
