Description

Schedule/Availability Requirements:

7-8 call shifts/month, can be combined with clinic days

3 days of clinic per week, 3 weeks per month preferred

Multiple part time locums accepted to fill schedule: Site prefers just one for clinic consistency, willing to consider locums for just call or clinic.

 

Shift Details: 24 hr call 7a-7a with 20 min response time. Contact by cell phone. Clinic- 8a-5p Monday-Friday.

Setting: Critical Access Hospital and Clinic

Duties: OB/GYN clinic care, OB and GYN call coverage through L&D Unit and ED.

Required Procedures: C-Section, Ectopic

EMR: Epic

Support Staff: MA or LPN in clinic. L&D RNs at hospital

Reason for Coverage: Medical Leave

Submission Requirements

**MUST NOTATE ALL DURING NAME CLEARS**Board Certified/Board Eligible – Required

WA Active license/IMLC considered (LOQ must be uploaded or submission will be rejected) – Required

Clean Malpractice – Highly Preferred

BLS/ACLS/DEA – Required

Availability at time of name clear(Specific dates and black out dates required for presentation) – Required

Availability for orientation(Specific dates and black out dates required for presentation) – Required---
Vendors are now required to compile all presentation documents for a locum into a single PDF file.

The PDF must be uploaded into TRIO in the "Candidate Documents" section under the locum's submission page.The PDF title should include the locum's name, site location, and specialty (ex. Dr. John Smith Good Samaritan Physician Nephrology)An up-to-date CV (solely) is still required to submit in “Name Clear Documents” initially when sending a new name clear. Vendors will have until bid due date to submit a PDF of ALL presentation documents. If PDF is not submitted, TRIO will not clear provider. Required Documents for a Full Presentation Packet:Up-to-date CV (Gaps greater than 30 days will need notation/explanation of gap in work history)Malpractice documentation (NPDB and or write up). Must include ALL cases, dates, and payouts.

Supporting certifications such as DEA, BLS, ACLS, ATLS, NRP, PALS

Copy of clean Sex Offender search
