Description

RATE CAPS FOR SUBMISSION:

Malpractice
– Must be included in rates.

Orientation
– half the hourly/callback rate, kicks in after 8 hours. “Per hour of
orientation or training beyond 8 hours”

Schedule/Availability Requirements:

  • Monday-Friday 7a-5p (hourly), 5p-7a (night call - 2.5 gratis hours)
  • Sat-Sun - 7a-7a - 24 hour call (8 gratis hours)
  • Dec 27-Jan 6 (7a)

Details:

  • Facility Location: 1 Mercado St, Suite 110, Durango, CO 81321
  • Setting: Inpatient, Outpatient, Call, Clinic
    FTE: 1.0
  • Required Procedures/Job Duties: Interventional Call, Interventional procedures, both scheduled and unscheduled. Some clinic days depending on the length of the stay. Inpatient consults, and inpatient study reads as well.
  • EMR: EPIC
    Support Staff: RN
  • Reason for Coverage: loss of provider during credentialing.

 

EMR Required Training: 

  • Module (learn lessons) – Must be completed prior to providers first day.
  • EMR Onsite training – Only able to attend if Modules are completed before training. Provider will be scheduled for onsite EMR training if Modules are completed, the morning of their first day.

 

Submission Requirements

**MUST NOTATE ALL DURING NAME CLEARS, TO AVOID BEING REDIRECTED!!**
Board Certified Interventional Cardiology - Required

Active CO License - Required or IMLC (LOQ in hand at time of submission)

Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)

Residency Requirements/Fellowship – 2 years' experience minimum

Must disclose # of shift per month the provider can offer to the team ongoing - Required

Must be available for dates listed in job description to be considered for ongoing dates - Required

Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required

Clean Malpractice/background - Highly Preferred, case by case - Must include self inquiry NPDB report at time of presentation.
