Rate Caps for Submission:

Malpractice
– Must be included in rates.

Orientation
– half the hourly/callback rate, kicks in after 8 hours. “Per hour of
orientation or training beyond 8 hours”

Schedule/Availability Requirements:

Preferred schedule for locum:

Friday:  OR:  07:00- 15:00    1st Call:  15:00 - 07:00

Saturday:  OR:  07:00 - 11:00 (OR may be extended depending on volumes)   1st Call:  11:00 - 07:00

Sunday:  OR:  07:00 - 11:00 (OR may be extended depending on volumes)   1st Call:  11:00 - 07:00

In addition to weekend coverage, the following dates are also needed:

  • October 30 – Nov 3; 7a – 7a
  • Nov 25 – Dec 1; 7a – 7a
  • Dec 16 – Jan 5; 7a – 7a
